Position Summmary Provides diagnostic assessment, treatment planning, and outpatient therapy within identified patient population and provider's scope of practice. Collaborates within the BH Team, as well as with a healthcare team including medical, psychiatric, and community health staff. Responsible for maintaining an active caseload through scheduling, documentation, and communication practices that align with NHS policies and mission. Essential Functions Assess and diagnose patients with clinical proficiency and appropriate assessment tools consistent with DSM-5-TR and Minnesota DHS standards. Develop treatment plans in collaboration with clients that incorporate identified needs and goals, provider's clinical judgment and scope of practice, level of care assessment, and other relevant factors as appropriate. Provide ongoing outpatient therapy consistent with assessment and identified treatment goals. Maintain and expand a scope of practice which supports the needs of NHS's patient population, which may include but is not limited to proficiency treating patients with mood, adjustment, trauma, anxiety, substance use, thought, and/or co-occurring disorders who are assessed for an outpatient level of care and for whom the setting is conducive to the patient's treatment needs. Provide ongoing monitoring and review of patient progress and outcomes in response to treatment, including making changes to the treatment plan as possible and needed or preferred in collaboration with the patient. Plan and provide patient and family education as appropriate. Document patient care in the clinical record within the designated timeframe and in compliance with DHS and guarantor standards. Support patients with acute mental health needs when needed outside of scheduled appointments, directly or through other staff as appropriate, consistent with current integrated behavioral health models. Consult with manager, colleagues, external consultants, NHS policy, and/or professional ethical codes as needed or required to maintain sound clinical decision-making and alignment with professional and organizational expectations. Contribute to the efficiency and effectiveness of patient treatment and organizational functioning by participating as an active member of the patient care team. Participate in QI and quality assurance activities in clinical care areas as needed. Qualifications Active, unrestricted Minnesota license as an LPCC, LMFT, LICSW or LP preferred; may consider pre-licensed providers who have completed at least 6-months of clinically supervised practice. Master's degree or higher in psychology, counseling, social work, or marriage and family therapy. Minimum of two years' direct client care; experience in a primary care or collaborative treatment setting preferred. Demonstrated competence in the ethical, organizational, and clinical components of independent clinical practice. Experience providing care in multicultural practice setting required. Proficient in use of computer hardware and software, including Microsoft Office applications, to effectively document clinical care and communicate; willingness and capacity to become proficient in AthenaOne and other platforms. Skill-based pay available for demonstrated proficiency providing clinical services in Spanish. Compensation Range•$72,800•$83,200
