Job Description

Position Purposes :
The Behavioral Health Therapist provides affirming, respectful, and supportive care to individuals from diverse backgrounds and lived experiences. The Behavioral Health Therapist is responsible for completing initial mental health evaluations, providing individual, group, and / or family psychotherapy and counseling, and offering behavioral health consultation services. The Behavioral Health Therapist will work closely with staff across all departments and play a key role on the interdisciplinary health team for clients of Five Horizons Health Services (FHHS).
Essential Functions:
Provide clinical assessments, care planning, and effective treatment and interventions to clients Work collaboratively with clients using evidence-based approaches and treatment modalities Complete brief diagnostic assessments for mental health and other co-occurring disorders and provide diagnostic impressions / diagnoses Provide crisis assessments / triage with appropriate interventions and referrals Monitor and coordinate the delivery of services for clients as related to behavioral health, including linking with other treatment providers in the community Develop a therapeutic relationship with the client to enhance effective treatment delivery Assess clients' understanding and coping with physical, social, emotional, and medical issues Provide group psychoeducation / treatment related to chronic medical conditions Provide clinical consultation to medical providers and other staff regarding clients' mental and behavioral health and how it may be directly connected to clients' medical conditions Complete clinical documentation including assessments, treatment plans, progress notes, recommendations, discharge planning, and follow-up services Maintain compliance with HIPAA and state guidelines as well as FHHS policy and procedures As with all other positions, the Behavioral Health Therapist is expected to fill in at any position within the agency for which they are capable of performing.
Education/Professional:
Master's degree in Social Work, Counseling, Psychology, or related Behavioral Health field from an accredited institution Appropriate licensure through discipline-specific governing body to provide independent clinical services (LCSW, LPC, LMFT, etc.) Knowledge of evidence-based treatment modalities and behavioral health interventions for common mental health disorders Currently possess / have ability to obtain NPI and credentialing through CAQH Two years' experience providing mental health or substance use services preferred Working knowledge of differential diagnoses of common mental health and / or substance use disorders (DSM-5, ICD-10) Previous experience with telehealth / virtual services preferred
Physical Requirements:
This position requires an individual who is self-motivated and able to independently complete assigned responsibilities. The individual must also be able to accept proper supervision and work cooperatively with clients, peers, and other professionals. Frequent sitting, some standing / walking, and occasionally lifting supplies / equipment up to 50 lbs. Occasional reaching, bending, stooping, kneeling and crouching. This position requires an individual who is mentally alert and able to effectively communicate verbally and in writing. Individual must be able to effectively utilize Microsoft Office programming and, after necessary training, electronic health records system and Telemedicine equipment.
