$75,000 - $85,000 per year Job Details - The Behavioral Health Consultant serves as a member of the primary care team, providing behavioral health assessments, brief interventions, solution-focused therapy, crisis support, and care coordination. This role works closely with medical providers to address mental health concerns, substance use issues, chronic disease management, and behavioral factors impacting patient health. Services are delivered through an integrated care model designed to improve access, engagement, and patient outcomes. Key Responsibilities Conduct behavioral health assessments and diagnostic evaluations Provide brief therapy and solution-focused interventions Collaborate with primary care providers on treatment planning Support patients experiencing anxiety, depression, stress, trauma, and adjustment concerns Coordinate referrals to specialty behavioral health and community resources Maintain timely clinical documentation and treatment plans Participate in multidisciplinary care team meetings Provide crisis intervention and behavioral health consultation as needed Required Qualifications Active Pennsylvania LMFT, LCSW, or LPC license Master's degree in Counseling, Social Work, Marriage & Family Therapy, or related field Experience providing behavioral health counseling services Strong assessment, diagnostic, and treatment planning skills Ability to work collaboratively within a multidisciplinary healthcare environment Excellent communication and documentation skills A bit about us: - This growing community-based healthcare organization provides comprehensive medical, dental, behavioral health, and supportive services throughout Pennsylvania.
