Qualifications:
The Forensic Services Therapist provides individual and group therapy as well as care coordination and case management to individuals with Severe and Persistent Mental Illness (SPMI). Under the supervision of the Forensics Coordinator, the Forensics Therapist provides individualized direct care face-to-face and telephonic services to an identified caseload of consumers with serious and persistent mental health. This person works in collaboration with consumers, family members, and other service providers to assist consumers in gaining access to needed services and community resources. The goal is the promotion of consumers' recovery toward independence and community integration. This position requires a minimum of: a Master's degree in a human service field and a minimum of 2 years direct service with mental health consumers. It also requires a valid driver's license and a criminal history, child abuse clearance and FBI clearance. Strong organizational skills and oral/written communication skills are needed.
Duties/functions:
  • Maintain a primary caseload of 10-20 FSP consumers
  • Provides face to face individual therapy to forensic clients in their home environment which can include delivery of their medication.
  • Provides face to face group therapy in Moral Recantation Therapy (MRT) and Trauma Recover and Empowerment Model (TREM). Penndel Mental Health Center provides the funding for the therapist to be trained on these group models of treatment to be certified to provide these forms of group therapies after hire.
  • Conducts assessments of consumer strengths and behavioral health needs
  • Conducts treatment and safety planning with consumers
  • Participates in court proceedings with clients such as providing clinical updates to Bucks County Wellness Court.
  • Interfaces with probation/parole and the Bucks County Department of Behavioral Health.
  • Work collaboratively with consumer on caseload to develop individualized service plans based on assessments of strengths and needs
  • Assist consumers through the following community-based activities and as defined by individualized service plans
  • Linking with treatment and rehabilitation services as needed
  • Linking with community resources including housing, financial entitlements, employment, and recreational resources
  • Informal support network building
  • Crisis intervention/problem resolution
  • Consumer advocacy
  • Participates in daily team meetings and other meetings as required
  • Provides individual and group therapy to program participants both in the community and on site.
  • Completes required documentation in a timely and thorough manner
  • Maintains consumer records and activity reporting as required by team, agency policies and procedures and state regulations
  • Follows policies and procedures of the agency in all related administrative, clinical, quality improvement, and personnel areas
  • Seeks professional development and participates in required trainings
  • Maintain timely documentation of all work assignments and unusual incidents for risk management
  • Routinely keep supervisor informed about attendance and job assignments
  • Work cooperatively with peers
  • Performs related duties as required/assigned.
Physical Requirements:
This position normally requires the physical demands of standing, walking, bending, lifting, sitting on the floor or performing other work requiring low physical exertion, ability to move quickly in a crisis situation, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill.
