Job Description
Position Summary The Behavioral Health Integration (BHI) Psychotherapist provides brief, evidence-based behavioral health services through a collaborative care model integrated with primary care. This role is fully virtual and supports patients through telehealth while working closely with primary care providers, behavioral health teams, and our psychiatric consultants. The psychotherapist focuses on delivering short-term interventions, coordinating care, monitoring patient progress, and supporting group-based behavioral health programming. Key Responsibilities Collaborative Care Delivery Provide brief, structured psychotherapy services through telehealth for patients referred by primary care. Conduct behavioral health assessments and develop care plans in collaboration with primary care providers and psychiatric consultants. Deliver evidence-based interventions focused on symptom reduction and functional improvement within a short-term treatment framework. Monitor patient progress and adjust treatment plans as clinically indicated. Participate in regular case review with psychiatric consultants to support treatment planning and medication recommendations. Participate in interdisciplinary team meetings and collaborative case discussions. Care Coordination and Population Management Manage a panel of patients receiving collaborative care services and track progress and follow-up needs. Coordinate care between primary care providers, behavioral health teams, and specialty services when higher levels of care are needed. Support referrals to specialty behavioral health services when clinically appropriate. Provide follow-up contacts and bridge appointments as needed to support continuity of care and patient engagement. Integration with Primary Care Teams Collaborate closely with primary care providers and clinic staff to support integrated behavioral health care. Provide behavioral health guidance to primary care teams when appropriate. Group Facilitation Participate in the rotation of facilitating Behavioral Health Integration (BHI) therapeutic groups delivered virtually. Documentation and Quality Improvement Maintain timely and accurate documentation in the electronic medical record in accordance with organizational standards. Track patient outcomes and support reporting and quality improvement initiatives associated with collaborative care services. Contribute to ongoing program improvement efforts related to access, patient engagement, and clinical outcomes. Work Environment This is a fully remote/virtual position . All patient visits and group sessions are conducted through telehealth, and collaboration with care teams occurs through virtual meetings and electronic health record communication. The psychotherapist works closely with primary care clinics and behavioral health teams while operating remotely.
EDUCATION/EXPERIENCE REQUIRED
Master's (or Doctoral) degree in clinical or counseling psychology with one year supervised post masters experience; OR Master's degree in Social Work with two years post Master's experience in mental health/chemical dependency setting.CERTIFICATIONS/LICENSURES REQUIRED
Licensed Psychologist; OR Limited Licensed Psychologist; OR Licensed Master's in Social Work. Preferred Qualifications 2 years of Outpatient experience preferred. Experience working in integrated care. Familiarity with collaborative care or population-based behavioral health models. Experience delivering psychotherapy and group interventions through telehealth platforms. Skills and Competencies Ability to deliver brief, focused psychotherapy within a collaborative care environment. Strong collaboration skills when working with multidisciplinary healthcare teams. Effective clinical assessment and treatment planning abilities. Strong organizational skills for managing a patient panel and tracking outcomes. Comfort facilitating structured therapeutic groups in a virtual format.
