We are seeking a compassionate and motivated Licensed Mental Health Therapist to join our growing behavioral health team. Position Summary The Licensed Therapist will provide outpatient counseling services to adolescents and adults experiencing a variety of mental health concerns, including anxiety, depression, stress, trauma, relationship challenges, and life transitions. The ideal candidate values holistic, patient-centered care and enjoys working collaboratively within an interdisciplinary healthcare setting. Responsibilities Provide individual therapy in an outpatient setting Conduct comprehensive psychosocial assessments and develop treatment plans Support patients experiencing anxiety, depression, trauma, ADHD, stress-related conditions, and other behavioral health concerns Utilize evidence-based therapeutic modalities tailored to patient needs Maintain timely and accurate clinical documentation Collaborate with primary care providers, psychiatric providers, and interdisciplinary team members Participate in care coordination and treatment planning Foster a safe, supportive, and inclusive therapeutic environment Provide patient education and wellness-focused support strategies Qualifications Active Connecticut licensure as one of the following:
LCSW LPC LMFT
Strong clinical and interpersonal skills Experience providing outpatient therapy services preferred Ability to work collaboratively within a team-based care model Commitment to compassionate, patient-centered care Experience with trauma-informed care and integrative approaches is a plus Bilingual English/Spanish is a plus Training & Professional Development PATH is committed to supporting clinicians interested in whole-person wellness and integrative care.
