Job Description
Healthcare Staffing Professionals has an immediate need for a Healthcare Educator Assistant (Community Health Worker) to provide outreach and support to pregnant and parenting individuals and their families. In this role, we center equity and value community experience and lived experience as key skills. The CHW will engage clients through street outreach, home visits, and community-based engagement, provide resource navigation, link clients to services, and help build long-term, trust-based relationships. This position works collaboratively with Public Health Nurses, and other multidisciplinary teams to ensure clients have access to the programs and resources they need to thrive. The positions will start immediately upon acceptance and employment clearance and are initially funded through 6/30/26, with the good chance of extension.
Pay Rate:
$25.51/hr working 40 hours a week
Location :
Sylmar, CA | Field-based work requiring travel within assigned service areas. | Some evening or weekend hours may be required for community events or outreach activities.

Initial Job Duties:
Conduct street outreach and home/support visits across designated areas in Los Angeles County. Collaborate with Public Health Nurses and other team members to provide coordinated care and support for clients. Identify, screen, and recruit eligible clients to home visiting and family support programs. Build long-term, trusting relationships with clients and their families, affirming their lived experiences and cultural perspectives. Provide individualized support, referrals, and resource navigation, including mental health services, healthcare access, food assistance, parenting support, housing resources, and early childhood programs. Collaborate with community partners and agencies to strengthen client engagement and continuity of care. Respond to agency partner inquiries about program services and eligibility; provide information and referrals via phone or email as needed. Maintain accurate and timely documentation of outreach contacts, referrals, and service linkages in program data systems. Be trained in Motivational Interviewing and other relevant methodologies to support client readiness and engagement. Transport program materials, client documents, or supplies within assigned service areas as needed. Participate in additional outreach or community engagement activities as assigned. Adhere to Department of Public Health (DPH) and MCAH program policies, procedures, and confidentiality requirements.
Requirements:
Two years of paid or unpaid experience in the coordination, planning, or implementation of a health-related program -OR-Graduation from an accredited college with a major in a health-related field A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
Desirable Qualifications:
Lived experience or strong community knowledge related to supporting high-risk pregnant or parenting clients, particularly those experiencing homelessness or systemic barriers, is highly valued. Home visiting or community health work experience preferred. Strong verbal, written, and interpersonal communication skills. Ability to build rapport with clients from diverse cultural, linguistic, and socioeconomic backgrounds. Highly organized, flexible, and able to manage multiple priorities independently. Knowledge of and ability to connect clients to community resources to meet their health and social needs. Familiarity with equity-centered, trauma-informed, and strengths-based approaches to client engagement.
