Among the primary duties are:
Conduct comprehensive assessments of patients' dietary needs, medical history, and lifestyle factors to develop personalized nutrition plans tailored to their individual health goals and medical conditions, with a specific focus on diabetes management, and chronic diseases. Provide in-depth education and counseling to patients with diabetes and their families on topics such as blood glucose monitoring, carbohydrate counting, meal planning, medication management, and lifestyle modifications to achieve optimal glycemic control and prevent complications. Develop and administer curriculum for the Diabetes Teaching Kitchen Program. Use a pre‐approved curriculum and prepare presentations to promote healthy eating and nutritional information. Coordinate weekly nutrition classes, which include but are not limited to food purchasing and food preparation for scheduled program classes. Conduct outreach with non‐profits and educational institutions for participants and/or possible locations for off-site classes within grant-funded focus areas. Coordinate reporting and recordkeeping requirements for those served in the Diabetes Teaching Kitchen Program. Develop and administer curriculum for DSME and DSMT programs. Document the nutrition diagnostic statement in PES (problem, etiology, and signs/symptoms) and provide supporting evidence within the progress note.
Requirements:
Bachelor's degree (B. A./B. Sc.) from an accredited four-year institution in nutrition, health education, or a related academic field with related experience. Certification as a Certified Health Educator Specialist (CHES) is a distinct advantage. Applicants are required to teach health education classes, workshops, and related community outreach activities. An equivalent combination of training and experience may be substituted for the educational requirement. Must experience and knowledge as a health educator, including chronic diseases and other health topics.
