Job Description
Where You'll Work Founded in 1940, Dignity Health - Marian Regional Medical Center is a 191-bed, acute care, nonprofit hospital located in Santa Maria, California. Serving over 250,000 patients annually, the hospital offers a full complement of services including Level II Trauma Center, Level III NICU, cancer care, orthopedics, and stroke care. Additionally, Marian Regional Medical Center has been recognized as an LGBTQ+ Healthcare Equality High Performer by the Human Rights Campaign Foundation. It is a Joint Commission-certified Primary Stroke Center, and the only healthcare facility on the Central Coast to offer ECMO. One Community. One Mission. One California Job Summary and Responsibilities As our Community Health Worker, you will be a vital link between individuals and essential community resources, actively working to reduce health disparities and improve access to support services.
Every day, you will proactively engage with clients, assessing their needs and identifying pathways to care and support. You'll connect individuals to relevant health, social, educational, and employment resources, providing guidance and practical assistance to overcome logistical barriers.
To be successful in this role, you will possess a profound understanding of diverse community needs, exceptional problem-solving abilities, empathy, and resilience. Your dedication to empowering clients and removing barriers to care will be paramount. Maintains room and equipment in a safe manner. Reports any safety issue to program manager. Understands the basic learning requirements of the grade school, adolescent and/or adult learner. Demonstrates good instructional, communication and interpersonal skills. Uses appropriate age-specific language, written materials and visual aids. Provides instruction based on specified program curriculum 90% of the time. Attends department staff meetings as well as other meetings related for program over site.

High School Graduate General Studies, upon hire or High School GED General Studies, upon hire andDriver's License:
CA, upon hire and Auto Insurance, upon hire Above average interpersonal and instructional skills with grade school children, adolescents and adults. Good organizational skills Above average interpersonal, instructional and communication skills for K-12 and adults. Completion of employer provided basic course-specific training. Successful completion of employer provided training including demonstration of satisfactory of instructional skills with age-appropriate group.Preferred:
