  • Immediate Provider Opportunity in Fort Collins, CO area
  • Focus Care is looking for a part time/full time individual who will work as a mentor, coach and trainer to support individuals with learning and practicing the skills necessary to maintain successful life in the community and to avoid unnecessary institutionalization in a nursing home.
Life skills trainers will have a caseload of individuals with which they will create a care plan to outline the goals for working together. Each goal will have corresponding learning activities aimed at teaching the member the skills necessary to accomplish each independent living goal. Participants in the Life Skills Training (LST) program are eligible based on either a move from a skilled nursing facility into the community or by a qualifying life change. Life Skills Trainers take a person-centered approach to support each individual with accomplishing their personal independent living goals. This position involves working with the adult Medicaid population to assess, train and coach on a wide range of skills including:
  • Developing and maintaining monthly budget and bill payment
  • Utilizing technology to enhance care and ability to communicate with providers
  • Assistance with accessing and applying for benefits
  • Self-advocacy, self-care, anger management, coping with mental health issues
  • Transportation planning, time management and appointment scheduling
  • Meal preparation, development of menus, planning and shopping lists to support special diets. Learning how to access online shopping.
  • Support with maintaining healthy landlord - tenant relationships, adhering to lease
  • Accessing community resources to meet needs, opportunities for socialization to reduce isolation
  • Safety awareness and emergency planning
