at Christiana Care Health System in New Castle, Delaware, United States Job Description Job Details Are you ready to embark on an exciting journey with Christiana Care? ChristianaCare, with Hospitals in Wilmington and Newark, DE, as well as Elkton, MD, is one of the largest health care providers in the Mid-Atlantic Region. We are proud to that Christiana Hospital, Wilmington Hospital, our Ambulatory Services, and HomeHealth have all received ANCC Magnet Recognition. We are searching for a Community Educator to promote access to services, provide health education and assist community members in navigating the health care and social services systems. This position focuses on assisting pregnant and postpartum patients throughout their care journeys, which may include working with outpatient, OB Emergency Department, or NICU/Postpartum patients. Essential to the role of the Community Educator is the ability to handle competing priorities and operate independently. Additionally, they must be skilled at nurturing effective relationships, not only with patients, but also with members of the care team.
Key Responsibilities Develop and deliver health education and training programs for our patients and the community. Create connections between vulnerable populations, the health care system and community/state resources. Build and maintain positive working relationships with patients from diverse cultural and socio-economic backgrounds. Travel to patients' homes and in the community to discuss Community Health programs and provide services. Help patients identify socio-economic issues that affect their overall health and develop health/social management plans and goals. Facilitate healthcare and social service system navigation. Provide culturally appropriate health education on topics related to chronic disease prevention. Responsible for patient chart reviews and referring patients to other programs as appropriate. Assist patients in the community and/or clinic setting. Establish and maintain professional relationships with the care team, community leaders, faith leaders and others responsible for providing services to the community. Participate in community events promoting the Community Health department. Educate staff, health providers, and stakeholders about community needs. Advocates for underserved individuals to receive appropriate services. Works to reduce cultural and socio-economic barriers between clients and institutions. Collect data and updates Electronic Medical Record documentation as required by supervisor. Work collaboratively and effectively within a team. Establishes positive, supportive relationships with participants and provides feedback to other members of the team.

Qualifications:
High school diploma required, bachelor's degree preferred. One-year community and patient experience required, more preferred. Public speaking, group facilitation and excellent writing skills preferred. Demonstrated ability to work with diverse populations required. Fluency in Foreign Language and English requiredChristianaCare Offers:

- Knowledge and skills to provide education appropriate to target audiences.
- Foundational knowledge of substance use disorders and resources.
- Knowledge of community resources in New Castle County.
- Knowledge base in health education.
- Effective verbal skills to reach diverse racial, socio-economic and literacy levels of target audiences, strong interpersonal skills.
- Flexibility to adapt to a variety of program schedules, physical settings and target audiences.
- Ability to work with minimal supervision.
