06/01/2026 Total Compensation Estimator Tool- This is a fulltime Career Service (CS) Health Educator position working with the Department of Health in Lee County.
- This is a highly professional position with lead responsibilities for linking and re-engaging Human Immunodeficiency Virus (HIV) positive clients to care in the 7 counties of Area 8.
Programmatic Duties:
Use available databases (CARE Ware, STARS, HMS, Provide, the Linkage module etc.) to re-engage and link HIV positive individuals who are not in medical care, not taking anti-retroviral medication (ARVs), or have tested positive but have not been engaged into care. Contact individuals who are not in medical care, or who have been tested positive and are not aware of treatment options and community resources. The goal is to get the clients into medical care, initiate ARVs, and work collaboratively with case management support services to ensure continuum of care. Attend Statewide Linkage conferences and local case management meetings. Participate in Quality Assurance and Quality Improvement initiatives. Work with the local county STD Department to ensure that HIV and STD positive inmates are linked to care upon release from the county jail. Work closely with Area 8 surveillance officers to gain information on newly diagnosed HIV+ individuals, Area 8's medical and non-medical case management staff to identify and notify clients who are out of care from their case management database. This will also involve working with the Area Planning Groups such as the RegionalHIV/AIDS
Consortia (RHAC), the Prevention Planning Committee, the Continuum of Care Committee, and the Early Intervention Consultant (EIC) on patient care matters.Miscellaneous Activities:
Attend and participate in Linkage conferences, local case management meetings, general staff meetings and staff development, such as team/staff meetings and in-service/ continuing education. Perform related duties as necessary including public health services during a disaster. Maintain timely submission of EARs and timesheets. Performs other duties as assigned.Required Knowledge, Skills, and Abilities:
Knowledge of social, economic, rehabilitative or health care services, administration, or planning techniques. Knowledge of principles, practices & procedures Knowledge of methods of compiling, organizing, and analyzing data for the purpose of monitoring and evaluating Skill in operating a personal computer and software applications to include Microsoft Publisher, Microsoft Work and Microsoft Excel. Skilled in public speaking Skilled at listening to what other people are saying and asking appropriate questions. Skilled in written and oral communications Skilled at observing and evaluating the outcomes of a problem; solution to identify lessons learned or redirect efforts. Certified as anHIV/AIDS
