Diabetes Educator

Department Diabetes & OP Services Job Summary Responsible for guiding the education for patients of all ages with Diabetes and their family members on an outpatient basis. Functions as a Diabetes Educator cross-trained in the instruction of all aspects of diabetes. Required Qualifications

• Require current RN Florida licensure or RD with Florida licensure.
• Require Certified Diabetes Educator (CDE); or CDE eligible to sit for the exam when eligible; or obtains a minimum of 15 hours continuing education in Diabetes specific, diabetes related, psychosocial, educational, or program management topics each year.
• Require minimum of one (1) year of teaching or clinical experience. Preferred Qualifications
• Prefer academic preparation and/or experience in the care of patients with chronic disease and program management.

Bachelor Degree Required License and Certs Preferred License and Certs Our clinic sites rotate bewteen 3 locations at this time: Bee Ridge, BlackBurn Point, and Heritage Harbor.
