Behavioral Health & Outpatient Services Location:
Dubuque, IA (or relevant service area)
Department:

Hillcrest Family Services is seeking a dynamic and community-focused Health Educator to support screening, prevention education, and rapid linkage-to-care services for high-risk populations. This position is ideal for someone who enjoys community engagement, outreach events, and one-on-one education — and who believes prevention and access to care can change lives. What You'll Do Provide screening and same-day testing support for individuals at risk Deliver evidence-based education on prevention, risk-reduction, and health literacy Coordinate warm handoffs to treatment and care partners Support outreach efforts in shelters, treatment programs, correctional settings, and rural communities Participate in health fairs, wellness events, and awareness campaigns Track engagement, incentives, and service outcomes in alignment with grant requirements Maintain accurate documentation and assist with required grant reporting Collaborate with behavioral health providers, peer support staff, and medical partners What You Bring High school diploma or equivalent (Associate or Bachelor's preferred in social services or related field) 1+ year of experience in health education or community outreach Knowledge of high-risk populations and health disparities Strong communication and relationship-building skills Comfort working in both clinic and community-based settings Phlebotomy training preferred Ability to travel to outreach sites Certification as a Mandatory Reporter of Child/Dependent Adult Abuse is required within four months of hire.
