Job Description:

Siouxland Community Health Center has an opening for: RN Health Coach 1 - Full-time opening (hours TBD) Successful candidate must have solid experience & advanced knowledge of: Minimum of RN license, BSN preferred. Current BLS and Mandatory Reporter certifications. Clinical office and case management experience preferred. Certified Diabetes Educator certification or ability to obtain within 6 months of employment. Experienced RN with a background in case management who is self motivated and capable of working with others to improve patient care. Successful candidate must be able to perform primary functions of position: Educate patients on chronic disease management and prevention Assist patients with identifying and setting proper self-management goals including coordination to determine an appropriate goal achievement Contact insurance companies for preauthorization/precertification approvals for doctor's visits, procedures, diagnostic test, etc. Including maintaining detailed records in the EMR of all preauthorization activities. Instruct diabetic patients on proper use of diabetic meter, insulin instructions, and knowledge of downloading meter results Promote public awareness and education in the preventative health habits of SCHC patients via proactively assuring proper screenings are completed Collaborate with provider teams on a regular basis, ensuring to be an active part in daily huddles These functions are not all inclusive Siouxland Community Health Center provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to any characteristic protected by federal, state, or local laws.
