  • Chicago, IL (Loan Repayment Program) We are seeking a Diabetic Educator Registered Nurse to join a well respected Community Based Clinic serving the needs of the underserved community. The provider would work Mon-Fri (8:00am
  • 5:00pm) with an interdisciplinary team that is focused on improving patient outcomes within their diabetic patient population.
The provider would work directly with the Medical Director and Clinic Manager that are fully supportive in this role. The Certified Diabetic Educator (CDE) performs nursing, consultation, and educational services for person with diagnosis of pre-diabetes and diabetes. In addition the CDE will provide community health education. Develop curriculum on health, and act as a health care consultant by providing information on the management of diabetes for patients of all ages from pediatric to adult patients.
DUTIES & RESPONSIBILITIES
  • Provide outpatient diabetes, and self-management training and education to type 1 and type 2, and gestational diabetic patients. Education includes, but is not limited to signs|symptoms and respective treatments for hyperglycemia, hypoglycemia, DKA, and sick day therapies.
  • Plan, develop, and implement individual and group diabetes education for adults, pediatrics, and their families and caregivers.
  • Instruct patients in the standards of care for diabetes.
  • Provide nutritional counseling for weight control and disease management.
  • Develop, plan, and implement focused diabetic days to increase patient compliance at primary care sites.
  • Develop collaborative relationships with other departments, services, and community health care agencies to facilitate and support quality diabetes care.
  • Attend outreach activities such as health fairs for diabetes screening and health education.
  • Evaluate the diabetes and health education program on a continuing basis; assesses teaching methods and education material for staff, patients and families of patients.
  • Monitor and evaluate short and long-term patient responses to therapeutic interventions in collaboration with providers; provides and maintains necessary follow-up for patients and families.
  • Track data of patient care to include number of patients seen, demonstrating patient progress to goals.
  • Train and educate nursing staff in diabetes and health clinical procedures, coordinates in-service training, continuing education, and other related diabetes and health clinical training.
  • Instruct patients and family members on how to administer insulin, self-monitor blood glucose, and medication education.
  • Maintain professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise.
  • Perform all related duties as assigned or requested.
KNOWLEDGE, SKILLS, & ABILITIES
Professional knowledge and skill as a diabetes and health educator. Excellent communication skills [verbal & written], critical thinking, and demonstrates keen problem solving skills. In Depth knowledge and understanding of the nature of diabetes and related issues. Ability to design, develop, and deliver educational programs and materials.
QUALIFICATIONS
Registered Nurse or other Health Professional Credentialed as a CDE One year of experience as a CDE preferred but not required Maintains credentialing as required by certification body.
