Summary Job Description:
Admissions Facilitator BHCOR-Cor Rehab Full-Time | 1st Shift BHCOR-Rehab Program I Corbin, KY The Rehab Program is the Operations side of the Rehab Unit. Positions in this department include discharge planning, case management, admissions, and therapeutic recreation. Coordinates placement of all patient admissions (elective and emergent) as well as patient transfers in accordance with clinical standards and guidelines. Promotes a sense of organizational ownership by managing change and improving patient throughput. Assures appropriate patient flow by collaboration with various departments, MD offices and outlying facilities. Ensures the patient is admitted to the most appropriate unit/ bed available using criteria for admission as well as any identified special needs.
Description:
Manages hospital bed resources and coordinates patient placement based on clinical needs, level of care, and bed availability. Collaborates with nursing units, house supervisors, physicians, housekeeping, and patient flow teams to optimize patient throughput and bed utilization. Monitors hospital census, bed status, admission delays, and staffing impacts to support efficient patient flow. Serves as a central point of contact for patient admissions, bed assignments, and physician communication. Identifies and resolves bed placement issues, complaints, and process barriers while implementing continuous improvement initiatives. Utilizes performance improvement and Six Sigma principles to enhance bed management and admission processes. Coordinates patient admissions by reviewing clinical information, ensuring appropriate admission criteria are met, and facilitating communication among care teams. Supports operational planning, scheduling, and resource allocation to maintain effective patient flow throughout the organization.
Requirements:
Bachelor's degree in Nursing (BSN) or equivalent combination of education and experience required. Minimum of three years of direct clinical experience in an acute care hospital required. Current RN licensure required. Telemetry experience required. Critical care experience preferred.
