Job Description

Lifestyle Coach Program Our coaches teach the updated 2021 CDC National Diabetes Prevention program. This program is a national evidenced based program led by the Centers for Disease Control and Prevention (CDC). The program is a yearlong program. The core classes (First 4 months) meet once per week. The post core (last 8 months of the program) meets bi-weekly (2 months) a then transition to monthly (6 months). Our CDC-trained coaches are also trained initially internally by the coordinator (unpaid). The role of the Lifestyle Coach is to facilitate the yearlong class using the provided evidence based curriculum. Some of the responsibilities of the coach include: support participants throughout the program, collect data such as weight and physical activity, input data into the system on a regular basis, review food trackers and logs, and the use of creativity to keep participants engaged and successful in the program. The Lifestyle coaches will assist participants with healthy eating and increasing physical activity throughout the program.
We are seeking candidates who:
Professionally trained and hold certification as CDC
  • Diabetes Prevention Program Lifestyle Coaches ( This is a requirement and proof of completion is required) Are experienced health care professionals, health care educators, health care coaches with 15+ years experience Active Health/Wellness Coaches Are critical thinkers that are great at problem solving and communication Have an ability to work with a diverse population Proficient at using the WebEx and Zoom video platforms Have the ability to coach one or two 60-minute sessions weekly Located locally to coach in person (MUST be located within Prince George's County, Maryland) Understand that this is a part time position best suited for a candidate with evening and/or weekend availability We encourage those who are energetic, retired, or those seeking PT work to apply This is a paid position with a 1 year commitment required.

Please be prepared to provide your training organization and completion date for verification. We work at various site locations to provide the program throughout Prince George's County, Maryland. Coach must reside within or nearby Prince George's County as classes are transitioning back to in-person.
