Then you would love to become a Health Educator, full time position (40 hours/week) for Isabella County at the Central Michigan District Health Department, working in the Community Health Department. This position plays a vital role in Human Immunodeficiency virus (HIV) and syphilis prevention, screening and linkage to treatment. Duties include providing HIV partner services and intervention through client interviews and community visits, documentation and case reporting utilizing the Michigan Disease Surveillance System (MDSS) and Aphirm as well as provider outreach and education regarding syphilis treatment recommendations. This Health Educator will also be responsible for HIV and other sexually transmitted infection (STI) testing through in office clinic days and outreach events, Travel will be required with reimbursement. A Bachelor's Degree in Health Education, Nursing, or Social Worker required. Some travel will be necessary. Phlebotomy experience desired and CPR certification required. General Summary - Responsible for planning, developing, leading and evaluating multiple health promotion programs for specialized area or population. Develops and facilitates community health coalitions or work groups. Serves as professional resource to other health educators and assists in monitoring and guiding program methods and practices to ensure effective outcomes. Provides consultation to community organizations, worksites and schools in developing and implementing effective programs and services. Essential Duties - Plans, develops, implements, leads and evaluates health promotion programs by determining priorities and strategies in consultation with supervisor and community partners and stakeholders. Responsible for organizing and providing leadership in a specialized area of public health and/or specialized population for multi-county area, or on an agency-wide or regional level. Develops strategic partnerships and facilitates group process around data to drive systems change to improve population health through collective impact. Works collaboratively to develop, facilitate or participate in community health coalitions or work groups to support identified health improvement programming. Identifies community health needs and assets through primary and secondary data sources. May participate in implementing Community Health Needs Assessment process. Provides consultation in the development of specialized health education/promotion programs and intervention. Develops and maintains culturally appropriate health education information and educational resources for use by individuals, community groups, schools and internal health department units. Assists with strategic planning, implementation and monitoring specific program assignments. Conducts basic evaluation of community health program outcomes and results and prepares related reports. Provide program specific health screenings, exposure assessments, and risk reduction counseling to improve health outcomes at an individual and community level. Deliver evidence-based interventions and education to individuals within an agency-wide area. Identify, coordinate, and facilitate referrals for individuals and/or community organizations to appropriate resources to promote positive health outcomes. Other Duties & Responsibilities - Participates in training to maintain professional excellence in the field of Public Health. Maintains confidential information. Performs all other duties as assigned or in response to emergencies, workload or staffing shortages. Maintains up-to-date vaccines including flu (influenza,) in accordance with CDC guidelines: Recommended Vaccines for Healthcare Workers, in which you allow CMDHD to access your current immunization status based on Michigan Improvement Care Registry (MCIR). Minimum Qualifications - Advanced knowledge of community health theories, strategies, and evaluation methods, at a level normally acquired through completion of a master's degree in health promotion, public health or related field; or Bachelor's degree in health education/promotion, community or school health, public health or related field with three years' experience in community health or public health field. Demonstrated competency and ability to independently plan, develop and implement specialized health promotion programs at a level normally acquired through three years progressively more responsible, related experience. Commitment to professional development which may be shown through attainment of recognized certifications. CHES or MCHES certification preferred. Depending on funding source or program standards, CPS certification or ability to acquire within defined timeframe may be required. CPR certification may be required depending on program assigned. Basic understanding of computer technology and social media and their application to health education practices. Ability to work effectively with the public and other employees on projects requiring a team approach; and take leadership role and mentor new health education staff Must work independently to accomplish established work plans seeking assistance as needed. Must have emotional and mental maturity necessary for establishing and maintaining good working relationships with clients, community partners and agency personnel. Good written and oral communication skills; excellent public speaking and group facilitation skills. Knowledge of and ability to devise and conduct data analyses and reporting. Ability to accurately record services and data within established time frames. Reliable transportation with valid Michigan driver's license and certificate of insurance to travel to other locations for work, training, or meetings. Completion of National Incident Management System/Incident Command System online and face-to-face training courses based on current Public Health Emergency Preparedness requirements. Environment & Effort - Depending on program assigned, works inside in general office or clinic environment, or school, community or worksite setting. Exposure Control - Category A. At higher risk for occupational percutaneous or mucosal exposure to blood or body fluids. Per OSHA's Bloodborne Pathogens standard requirements, CMDHD protects the employee by an established exposure control plan which includes but is not limited to use of universal precautions, engineering controls, work practice controls, personal protective equipment, and worker training. Some exposure to adverse weather conditions when traveling to other locations or conducting programming. Physical effort necessary to move about office, clinic, worksite and/or community environments; lean and bend to provide programming; travel to programming responsibilities; and occasionally lift or move objects weighing up to 40 pounds. Manual dexterity and feeling by touch necessary to pick, pinch, and palpate; and grasp and use small instruments. Visual effort regularly required in reading and reviewing records and reports; preparing and analyzing data; viewing computer screens; reading scales, equipment screens, and gauges; and performing close contact procedures such as finger stick for blood analysis and obtaining vital statistics.
