Full Job Description Job Summary:
Momba Home Care Michigan is seeking a Program Manager & Health Coach to fill a dynamic dual role. You will oversee the full caregiver recruitment process — sourcing candidates, conducting interviews, negotiating pay rates, and supporting onboarding — while also working directly with individuals with disabilities to develop personalized care plans and guide them in achieving their daily activities.
Hybrid Role:
Onsite & In the Field (Travel is required)
Full-time:
Monday-Friday 9AM-5
PM Salary:
60K-70K Must be a Licensed Social Worker (Required) Healthcare Operations Management Experience (Required) Experience with Home Care, Managed Care Organizations (MCO's), and Area Agency on Aging (AAA's) (Required) Must be a self-starter who is hungry for growth!
Qualifications/Educational Requirements:
1.
Education:
Bachelor's degree in Nursing, Healthcare Administration, or a related field. Certification in healthcare management and/or health coaching preferred. 2.
Experience:
3-5 years of experience in healthcare management, with a focus on home health or similar settings. Proven experience in a health coaching role, preferably in home care. 3.
Program & Care Plan Knowledge:
Familiarity with Medicaid Waiver programs and their eligibility criteria is advantageous. Demonstrated ability to develop personalized care plans based on individual client needs. 4.
Communication Skills:
Excellent verbal and written communication skills to effectively interact with clients, caregivers, healthcare professionals, and internal teams. Ability to build strong rapport with clients and families. 5. Attention to
Detail:
Strong organizational skills with an eye for detail to accurately collect, document, and record client information. 6.
Empathy & Compassion:
Demonstrated ability to empathize with clients and their families while maintaining professionalism and confidentiality throughout all interactions. 7.
Adaptability:
Ability to thrive in a fast-paced environment and manage multiple tasks simultaneously while maintaining a positive, solution-oriented attitude. 8.
Problem-Solving:
Strong critical thinking skills to assess client and caregiver needs and provide timely, appropriate solutions and coaching support. 9.
Technology Proficiency:
Proficient in electronic health records (EHR) systems and the MS Office suite. Comfortable using mobile applications for care coordination and coaching. 10.
Ethical Standards:
Commitment to high integrity, adherence to ethical standards, and strict compliance with HIPAA confidentiality guidelines.
Responsibilities/essential functions:
1.
Client Care Planning:
Collaborate with clients, caregivers, and the clinical team to create achievable, personalized care plans that support sustainable health improvements. 2.
Monitor & Adjust Care Plans:
Monitor client progress on an ongoing basis and adjust client-specific care plans and caregiver tasks as needed to ensure optimal outcomes. 3.
Client & Community Relations:
Build and maintain positive relationships with clients, case managers, and community partners. Conduct semi-annual home visits (and as needed). Market the company's services and increase community awareness. 4.
Source & Attract Candidates:
Proactively identify, source, and engage qualified candidates through job boards, social media, referrals, and other channels to meet the organization's ongoing hiring needs. 5.
Manage Recruitment Process:
Oversee the end-to-end recruitment process, including job postings, screening, interviews, and offers. Conduct phone, video, and in-person interviews to assess qualifications, culture fit, and job-specific competencies. 6.
Build & Maintain Pipelines:
Develop and maintain a pipeline of caregiver candidates to meet both current and future hiring demands of the organization. 7.
Onboarding & Caregiver Support:
Negotiate pay rates with prospective caregivers and collaborate with internal teams to ensure seamless onboarding. Provide ongoing support to resolve caregiver issues or concerns. 8.
Candidate Experience:
Ensure a seamless and professional experience for all candidates from initial contact through onboarding, enhancing Momba Home Care's employer brand. 9.
Territory & Business Development:
Regularly travel within the assigned territory to meet with clients and prospects, fostering face-to-face relationships and expanding business opportunities. 10.
Reporting & Metrics:
Track and report on recruitment metrics to assess the effectiveness of hiring strategies. Report progress daily to the supervisor and attend daily meetings to present caseload information. 11.
Documentation & Data Entry:
Ensure timely data entry and upload of new client packets. Complete timely incident reporting in accordance with company procedures. 12.
