The PATH team locates and identifies disabled and mentally ill persons who are homeless to help with transitioning to a more stable environment and out of homelessness. This is accomplished through support, assistance, referrals, and crisis care. We also provide advocacy, encouragement, empowerment and normalization to bring successful re-entry into the community. Schedule is Monday - Friday, 8:00 am - 4:00 pm. Prefer Bachelor's degree in a human services field with at least one year of full-time experience in a mental health setting. Requires excellent verbal and written communication skills, with ability to do detailed paperwork utilizing an electronic medical records system. Must have a valid driver's license and a working knowledge of community services.
