Job Description

•Req Number•189703 Job Description Coordinates and identifies community members needing information about HIV testing, treatment and prevention and Sexually Transmitted infection testing, treatment and prevention. Works with community agencies, does HIV/STI testing in a mobile van. Schedules patients for initial appointments. Works in the community and in the outreach office. Additional language skills are appreciated but not required. Provides health education and resources. Contacts and schedules patient/client's appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports. Job Responsibility + Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests. + Collects and organizes data required for statistical reports and completes reports, as required. + Attends and participates in meetings as appropriate. + Sets-up files and maintains records. + Provides incoming and in-process orientation. + Based on position, may be required to provide bi-lingual translation and/or guidance. + Performs related duties as required. Job Qualification + High School Diploma or equivalent required. + 0-1 years of relevant experience, required. + Previous experience in
HIV / HIV
prevention case management, preferred. + Bilingual - fluent in Spanish, preferred. + Ability to communicate effectively while being sensitive to cultural diversity. + Prior experience with the LGBTQ+ community, preferred. •Additional Salary Detail The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
