This job description may be modified as organizational needs dictate and changes will be discussed and documented. Department & Purpose Programs Responsible for fulfilling LADD's mission to empower adults with developmental disabilities to live, work and connect. Engages directly with developmentally disabled adults and their families based on their prescribed individual needs.
Shifts Available:
Monday, Friday, Saturday 8am to 4pm, Tuesdays 10am to 6pm.

Families Employee Classification Non-Exempt Summary The Health and Wellness Direct Support Professional provides wellness-focused support services to individuals with developmental disabilities. The Health and Wellness DSP fosters a teaching relationship that encourages healthy and productive personal growth, community inclusion, creativity, and independence. Essential Functions Assist individuals to engage in healthy initiatives (including teaching, oversight, and hands-on assistance) by ensuring individual's wellness goals are followed and documented as required. Encourage individual's engagement in social and community activities as well as appropriate physical/wellness activities. Know and be able to communicate effectively with the person receiving services. Ability to stay calm and proactively manage crisis and behaviors. Identify obstacles and help the person served problem solve around them. Ensure accurate and timely documentation. Meaningful collaboration with peers to ensure outcomes are met. Maintain flexibility to meet the health and wellness goals of individuals being supported. Build healthy habits and have a positive can-do attitude. Assist individuals in choosing activities important to them. Ensure environment meets safety requirements and address safety issues timely and appropriately. Communicate timely with manager regarding pertinent issues and work collaboratively with peers and other departments. Model The LADD Way (TLW) ensuring performance is aligned with TLW and the policies and practices set for the department. Represent LADD in a positive manner as appropriate. Keep up-to-date on all relevant training and certifications. Additional Responsibilities Participate in annual MyPlan meetings when able. Education, Skills & Experience High school diploma or equivalent required. Current CPR/First Aid Certification. Current Medication Administration Certification. Valid driver's license with fewer than six points, and be insured (if required). Ability to navigate various technology software and platforms. Work Environment & Physical Demands Work schedule will vary based on individual availability and will primarily include evenings and weekends. Job may include lifting and other physical activities. Job will include being in a residential environment and will require travel to other locations. ```
