• Experience is a must Responsibilities Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to develop personalized care plans.

Manage and coordinate ongoing care for patients with chronic illnesses such as diabetes, hypertension, heart disease, and respiratory conditions, ensuring adherence to treatment protocols. Provide health coaching and education on medication administration, lifestyle modifications, nutrition, and disease management strategies. Monitor patient progress through regular follow-ups via telehealth or in-person visits; adjust care plans as needed based on clinical findings and patient feedback. Experience Familiarity with EMR/EHR systems such as Epic, Cerner, eClinicalWorks or Athenahealth is essential for documentation efficiency. Knowledge of ICD coding (ICD-9/10), CPT coding, and medical terminology enhances clinical accuracy. Prior experience managing patients with complex needs including geriatrics (memory care, dementia), behavioral health issues, or developmental disabilities is preferred. Skills in triage assessment and acute pain management are valuable assets for this role. Strong physiology knowledge combined with excellent communication skills will enable you to educate patients effectively while fostering a supportive environment for their health journey.
