This position supports patients across our Long Island locations via phone and Zoom, providing individualized health coaching, behavior change support, and lifestyle intervention as part of an integrated, patient-centered care model. The ideal candidate brings a functional nutrition or integrative wellness lens to their practice and has experience working with patients managing chronic conditions including diabetes, hypertension, obesity, CKD, and metabolic health concerns. Key Responsibilities Deliver one-on-one health and wellness coaching sessions via phone and Zoom Apply motivational interviewing and evidence-based behavior change techniques to support sustainable lifestyle modification Develop individualized care plans focused on nutrition, movement, stress, sleep, and other lifestyle factors Collaborate with physicians, nurse practitioners, and other care team members as part of an interdisciplinary approach Document patient encounters accurately and in a timely manner within the EMR system Support patients managing chronic conditions including diabetes, hypertension, CKD, obesity, and metabolic disorders Provide patient education and develop health literacy resources as needed Participate in quality improvement initiatives and team meetings Required Qualifications NBC-HWC (National Board Certified Health & Wellness Coach) certification OR Wellcoaches certification Experience providing health coaching, behavior change support, motivational interviewing, and patient-centered goal setting Strong communication and patient engagement skills Ability to build rapport and support patients through sustainable lifestyle change Experience documenting patient encounters in an electronic health record (EHR) Ability to work independently while collaborating with interdisciplinary healthcare teams Preferred QualificationsNutrition & Wellness Certifications Certification in nutrition, functional nutrition, wellness, or a related field Functional Nutrition Alliance - Functional Nutrition and Lifestyle Practitioner (FNLP) Nutritional Therapy Practitioner (NTP) Integrative Nutrition Health Coach (INHC) Board Certified in Holistic Nutrition (BCHN) Registered Dietitian / Registered Dietitian Nutritionist (RD/RDN) or Certified Nutrition Specialist (CNS) Other evidence-based nutrition or wellness certifications Preferred Experience Weight management and lifestyle behavior change programs Chronic disease management, including CKD, diabetes, hypertension, obesity, and metabolic health Patient education and resource development Telehealth and remote patient engagement About PRINE Health Medical Group PRINE Health Medical Group is a multi-specialty medical group serving patients across approximately 23 locations on Long Island, NY.
