Full Job Description Job Title:
Integrative Health Coach Job Overview We are seeking a highly skilled Integrative Health Coach to join our clinical team and support patients in achieving sustainable improvements in metabolic health, lifestyle behavior, and overall well-being. This role works closely alongside our providers to help implement individualized care plans rooted in functional and integrative medicine principles. The ideal candidate is either an NBC-HWC (National Board Certified Health & Wellness Coach) or has equivalent advanced training from a reputable program, with demonstrated experience in functional medicine, nutrition science, nervous system regulation, and/or behavior change coaching. This is a clinically integrated coaching role that requires a strong understanding of nutrition protocols, therapeutic lifestyle interventions, and the ability to translate provider recommendations into actionable patient support. Responsibilities Partner with providers to implement and reinforce individualized patient care plans Conduct initial and ongoing health coaching sessions (in-person and virtual) Perform comprehensive lifestyle and wellness assessments (nutrition, stress, sleep, movement, behavior patterns) Support patients in behavior change using motivational interviewing and evidence-based coaching strategies Educate patients on nutrition strategies, dietary frameworks (e.g., elimination diets, low glycemic, anti-inflammatory approaches), and foundational wellness principles Provide guidance on stress regulation and nervous system support techniques Track patient progress, adherence, and outcomes through detailed clinical documentation Collaborate with the clinical team to adjust plans based on patient response and provider direction Maintain strong knowledge of clinic therapies, protocols, and supplement regimens Requirements NBC-HWC certification preferred OR formal education in health coaching, nutrition, exercise science, or related field from a reputable program Minimum 1-3 years of experience in health coaching, functional medicine, integrative health, or behavior change-focused care Strong understanding of nutrition science, metabolic health, and lifestyle medicine principles Experience working with structured dietary protocols and patient adherence support Familiarity with nervous system regulation, stress physiology, or mind-body modalities Skilled in motivational interviewing and patient-centered communication Ability to work collaboratively in a fast-paced clinical environment Strong organizational skills and attention to detail in clinical documentation Passion for patient education and empowerment Preferred Qualifications Experience in functional medicine or integrative primary care settings Background in exercise science, personal training, yoga, or somatic modalities Experience supporting supplement protocols and provider-directed treatment plans Familiarity with EMR systems and clinical workflow environments What You'll Be Part Of A collaborative integrative medicine primary care practice focused on addressing root causes of metabolic, hormonal, and chronic health conditions. You will play a key role in bridging clinical care and patient implementation to improve outcomes and long-term behavior change.
