Summary:
The Wellness Coach will conduct assessments to understand patient's unique needs, goals, and current health status. This role will gather information through interviews, questionnaires, and collaborate with other healthcare professionals to help ensure the wellness of LDH patients. What You'll Do Design personalized plans , tailored to the patient's preference, abilities and circumstances, that encompass various aspects of wellness, including physical activity, nutrition, stress management, sleep, and overall lifestyle choices. In collaboration with LDH clinical team, promote and reinforce behavior changes and assist with the development of effective self-management. Provide guidance, support, and education to patients, empowering them with the knowledge and tools to make healthy lifestyle changes. Assist with workshops, seminars, or one-on-one exercise classes at LDH Wellness Center. Track patient's progress, monitor their adherence to action plans and provide feedback while they are working with us. Perform regular check-ins, reassessments, and evaluation of outcomes which will allow coaches to adjust strategies. Utilize motivational interviewing techniques to elicit clients' intrinsic motivation for change. In collaboration with the multidisciplinary team, assist in monitoring and implementing LDH clinical protocols including, but not limited to the standardized visits, procedures and established quality metrics. Any other duty as assigned that would aid in wellness of the patient. What We're Looking For Bachelor's degree in health sciences required. Bilingual (English/ Spanish) preferred. At least three month's internship rotation as a Lifestyle/ Wellness coach assisting patients with cardiometabolic conditions.
ADA/ CHW
certification required. BLS certification is required. Excellent communication skills, interpersonal skills and professional phone etiquette required.
Physical Requirements:
Must be able to use force for up to 20 lbs. frequently May require use of up to 10 lbs. of force constantly to lift, carry, push, pull, or otherwise move objects, including the human body Speak clearly and loudly enough to be understood by Providers, peers, or patients in clinical settings, or on the telephone. Hear and retain pertinent information to relay and follow instructions from Providers, peers, or patients in clinical settings.
