Public Health Educator Nashville, TN $25 - $30 an hour $25 - $30 an hour One of our clients in the government domain is seeking a Public Health Educator to play a vital role in supporting individuals living with dementia and their caregivers by connecting them to community resources and clinical services. This position focuses on promoting dementia-friendly practices, increasing awareness of Alzheimer's disease and related dementias (ADRD), encouraging early detection, and enhancing access to care. Working within the Office of Healthy Aging, this role collaborates closely with healthcare providers, community organizations, and caregivers to ensure individuals with dementia can safely remain in their homes for as long as possible. Public Health Educator Hybrid 1216 Trotwood Ave, Columbia, Tennessee 38401 11 Months Key Responsibilities Community Outreach & Engagement Build and maintain relationships with community partners to support individuals with ADRD and caregivers Deliver educational presentations, memory screenings, and outreach programs Promote dementia-friendly environments and risk reduction strategies Collaborate with healthcare providers to improve early detection and individualized care planning Distribute evidence-based educational materials and resources Program Coordination & Data Management Track client outcomes, service utilization, and barriers to care Maintain accurate and secure records in compliance with privacy regulations Monitor program performance, outreach effectiveness, and client satisfaction Identify funding opportunities to support ADRD-related programs Develop and manage partnerships to enhance community support systems Knowledge & Professional Development Stay current with best practices related to dementia care, risk reduction, and caregiving Use data and insights to improve program delivery and outreach strategies Participate in ongoing training and professional development initiatives Collaboration & Communication Work closely with supervisors, team members, and external stakeholders Facilitate coordination between community organizations and healthcare providers Provide regular updates and ensure effective communication across all levels Compliance & Confidentiality Ensure strict adherence to HIPAA guidelines for protecting client information Manage financial and programming records in accordance with state and county policies Minimum Qualifications Bachelor's degree in public health, Nursing, Social Work, or a related field with 2+ years of relevant experience Licensed Practical Nurse (LPN) with 5+ years of healthcare/community experience Equivalent combination of education and experience (year-for-year substitution allowed).
