Job Description

POSITION SUMMARY
: The Certified Diabetes Educator is responsible for providing comprehensive diabetes self-management education and support to patients and their families. This role works collaboratively with physicians and nurse practitioners to empower patients with the knowledge, skills, and confidence needed to effectively manage diabetes and improve health outcomes.
KEY RESPONSIBILITIES
Provide individualized and group diabetes self-management education according to recognized standards of practice. Assess patient needs, barriers, and readiness to learn, and develop personalized care and education plans. Educate patients and families on nutrition, medication management, glucose monitoring, lifestyle changes, and complication prevention. Collaborate with providers to support patient-centered care and integrate education into the overall treatment plan. Support patients in behavior change, goal-setting, and problem-solving strategies. Maintain accurate documentation of assessments, education sessions, and patient progress in the electronic health record (EHR). Conduct community outreach, classes, and workshops on diabetes awareness and prevention as needed. Serve as a resource for clinic staff regarding best practices in diabetes care and education. Stay current with evidence-based guidelines, technologies, and advances in diabetes treatment and self-management.
QUALIFICATIONS
EDUCATION, EXPERIENCE
AND SKILLS REQUIRED
Bachelor's degree in nutrition, or a related healthcare field (Master's preferred). Current licensure as a Registered Dietitian. Certification as a Certified Diabetes Care and Education Specialist. Minimum of 2-3 years of clinical experience in diabetes care or patient education preferred. Strong communication, teaching, and motivational interviewing skills. Proficiency with electronic health records and diabetes-related technology (glucose meters, pumps, CGMs). This job description does not list all the duties of the job.
