Job Description

The Health Educator assesses healthcare needs and performs targeted healthcare education interventions. They serve as a referral source for providers and participate in community events.
Note :
This position is focused primarily on Annual Wellness Visits. Current licensure as a Registered Nurse or Licensed Practical Nurse OR Bachelor's Degree in health education or related field is required. Must demonstrate the ability to work with all life cycles in areas of health and lifestyle management. Requires close collaboration with medical providers and community organizations. A CDC Recognized Diabetes Prevention Lifestyle Coach and CPR Instructor Certifications must be obtained within six months of employment and maintained thereafter. Individual must be professional, self-motivated and have strong attention to detail, communication, teamwork, customer service and computer skills. Must complete and maintain all recertification requirements for continued licensure and/ or certification, and retain updated copies as applicable. Must demonstrate excellent internal and external customer service skills. Certification as an Interpreter is preferred and ability to provide bilingual patient care is a plus. Must have reliable transportation and a valid driver's license. A cell phone is required for this position. ESRHS will provide a monthly stipend towards cell phone reimbursement. Schedule a minimum of five (5) visits per day with patients in order to provide necessary education to improve patient outcomes, promote optimal management of chronic disease and perform Medicare Annual Wellness Visits. All Health Educators are required to schedule at least five (5) Medicare Annual Wellness Visits each week. The Health Educator assigned to Medicare Annual Wellness Visits is required to schedule at least twenty-six (26) Medicare Annual Wellness Visits each week Be a referral source to providers for patients. Attend a minimum of ten (10) community events to promote health and wellness on behalf of the organization. Document and follow-up on each patient education visit, whether by phone or face-to-face, in the electronic health record. Utilize a library of health education materials when appropriate. Facilitate the provision of BLS CPR to all required staff. Participate in quarterly Health Services Staff meetings to improve communication and working relationships with community partners. Review controlled medication agreements with patients as determined by the Controlled Medication Policy (C-POL-06), utilizing the template. Perform and maintain clinical skills in accordance with annual competency. Perform nursing skills (as determined by individual health educator's scope of practice) that are deemed necessary for health education visits and/or community health events (see health educator competency). Other duties and tasks as directed by the Health Educator Coordinator.
ADDITIONAL DUTIES ASSIGNED TO DESIGNATED BILINGUAL STAFF
If qualified, perform interpretation/translation services for ESRHS centers. If qualified, assist patients with signing up for the Migrant Clinician Network.
