Suicide Prevention & Wellness Specialist Job Description Helping Heroes is a community based, non-profit facility. It provides support services and resources to homeless and near homeless veterans in order to transition them to, or maintain self-sufficiency.
Job Title:
Suicide Prevention & Wellness Specialist Position Summary:
The Suicide Prevention & Wellness Specialist provides compassionate, client-centered support to individuals experiencing emotional distress, mental health challenges, and life transitions. This role combines crisis intervention, suicide prevention, health and wellness coaching, and adult life skills development to empower individuals to build resilience, improve overall well-being, and achieve personal stability and independence. This position will also assist with Health Care Navigation for all veteran clients. Services include connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care, case management and care coordination, health education, interdisciplinary collaboration, coordination, and consultation, and administrative duties. The Health Care Navigator will act as a liaison between the SSVF grantee and the VA or community medical clinic.
Key Responsibilities:
Suicide Prevention & Crisis Support Conduct risk assessments for suicide and self-harm using evidence-based tools Provide immediate crisis intervention, emotional support and follow-up coordination as needed. Develop and implement personalized safety plans Maintain accurate and confidential documentation of client interactions Collaborate with mental health professionals and emergency services when necessary Health Care Navigation Complies with SSVF (Supportive Services for Veteran Families) Health Care Navigator roles and responsibilities.
Including:
o Gaining access to health care o Supporting health care plans by identifying barriers to care o Providing education on wellness related topics Health & Wellness Coaching Assist clients in identifying personal goals, values, and purpose Support clients in developing healthy lifestyle habits (nutrition, sleep, stress management, physical activity) Teach through facilitating workshops or one-on-one sessions on practical life skills such as budgeting, time management, communication, and problem-solving Support independent living skills (housing stability, job readiness, daily routines) Provide psychoeducation on mental health and self-care strategies Support clients through life transitions such as career changes, recovery, or personal growth Use strength-based and solution-focused approaches Outreach and Collaboration Serve as the liaison to VA and community health care programs and represent the program in contacts with other agencies and the public. Develop and implement ongoing education plans for providers, Veterans, families, and community members on risk factors, warning signs, and treatment options for suicide Build and develop relationships with community leaders, VA staff, and other referral networks. Schedule meetings with referring agencies to explain program criteria Assist in conducting and coordinating outreach activities as assigned. Setting up and tearing down for events such as fundraising and outreach activities Coordinate care with community resources, healthcare providers, and social services Participates in interdisciplinary team meetings Stay current with best practices in suicide prevention and wellness coaching Interdisciplinary Collaboration, Coordination and Consultation Collaborate with other disciplines involved in providing care. Consult with other team members and appropriately assess and address the needs of the Veteran. Understand the different roles within the interdisciplinary team and act within professional boundaries. Adhere to ethical principles about confidentiality, informed consent, compliance with relevant laws, and agency policies (e.g., incident reporting, HIPPA, Duty to Warn). Administrative Duties and Systems Improvement Participate in expanding the knowledge related to health care navigators and the Veteran population. Help identify systemic barriers within the organization; communicate with organizational leadership about these barriers, and work collaboratively to find viable solutions. Participate in annual inspections conducted by funding agencies.
Qualifications:
Bachelor's degree in psychology, Social Work, Counseling, Health, or related field (Master's preferred) Experience in crisis intervention, coaching, or case management Knowledge of mental health, trauma-informed care, and behavior change principles Strong interpersonal, communication, and active listening skills Computer proficiency in Microsoft Office Suite Able to lift and carry 50 pounds
Preferred Skills:
Veterans of US Armed Forces preferred. Social Work License preferred. Training or certification in suicide prevention (e.g., ASIST, QPR) Certification in Health & Wellness Coaching or Life Coaching Experience working with diverse or at-risk populations Ability to maintain boundaries while providing empathetic support Strong organizational and documentation skills
Additional Requirements:
Submit to a criminal background check. Pre-Employment physical with drug screen. Copy of transcripts required upon hiring. Hold a valid driver's license, car insurance and be willing and able to travel. DMV driving record must be provided upon hiring.
Reporting:
This individual reports to the CEO who will provide supervision, assignments, and guidance to the Suicide Prevention & Wellness Specialist.
Pay/Benefits:
Hourly wage, retirement plan, employee health insurance.
Schedule:
Monday to Friday 8 am
  • 4 pm, with flexibility for evening/weekends as needed.
