Job Description

POSITION SUMMARY
The Health Information Management Clerk administers health information services consistent with accreditation and regulatory standards and facility standards. The Clerk implements policies and procedures for documenting, storing and retrieving information, processing medical legal documents, insurance data, and correspondence requests. The Clerk maintains patient medical records and ensures accurate documentation per requirements. The Clerk ensures HIPAA guidelines are followed.
ESSENTIAL FUNCTIONS
To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation. Abides by Standards of Ethical Code as set forth by HIPAA and adheres to those official guidelines. Provides assistance to the Director/Manager as delegated. Prep/Scan/Index and analyze all discharge records to ensure complete and accurate record is in the system and all required documentation and signatures are present. Adheres to release of information of patient medical records procedures according to hospital and regulatory requirements. Completes reanalysis of physician-revised records to ensure deficiencies have been completed. Ensure Fluency reports are correct and completed and placed in the medical record according to time requirements. Work collaboratively with hospital departments to process all revisions and requests for medical records. Informs/re-enforces HIPPA and 42 CFR Part 2 regulations and policies pertaining to medical records with all facility staff and requestors and stays current on all laws and regulations regarding confidentiality of patient/medical information. Assists with Performance Improvement Process for the department as directed by leadership. Additional Information:
Access to and/or works with sensitive and/or confidential information. Exhibit a comprehensive understanding of healthcare regulatory and compliance (e.g., HIPAA). Skilled in the application of policies and procedures.
KNOWLEDGE, SKILLS
The requirements listed below are representative of the knowledge, skills and/or abilities required. Knowledge in behavioral health care. Strong leadership and organizational skills. Ability to work collaboratively with a diverse team of healthcare professionals. Proficiency in using technology, including Microsoft Word, Excel, and PowerPoint. Excellent communication and interpersonal skills.
QUALIFICATIONS
High School Diploma/GED required. One year of health information management experience preferred. De-escalation certification required within 30 days of employment and prior to any patient contact.
