POSITION SUMMARY
Accountable for converting diagnoses and treatment procedures into codes using an international classification of diseases. This position requires skill in sequencing diagnoses/procedures to optimize reimbursement. The coder ensures that records are coded accurately and in a timely manner, maintaining patient medical records in compliance with regulations and standards. Duties also include filing, retrieving, and distributing medical records, maintaining associated logs, and preparing statistical reports in accordance with established policies and procedures in any section of the Health Information Management Department.
QUALIFICATIONS
  • High school graduate or equivalent.
  • Graduate of an accredited
AHIMA/CAHIIM
program.
  • CCA, CCS, or RHIT certification required.
  • 1 or more years of previous hospital experience as a Coder.
  • Knowledge of diagnoses/procedures in accordance with ICD-9-CM and DSM-IV.
  • Ability to collaborate with physicians.
  • Proficiency in English communication, both written and verbal.
  • Basic computer knowledge.
RESPONSIBILITIES
  • Analyze charts and enter Axis I-V information.
  • Ensure records are coded within 24 hours of discharge, excluding weekends and holidays.
  • Review medical records to identify all relevant diagnoses and procedures.
  • Contact responsible physicians professionally if diagnoses are not documented.
  • Ensure the coder's diagnoses and procedures match the physician's documentation 100% of the time.
  • Refer questionable records to the director for clarification.
  • Utilize computerized coding/abstracting equipment.
  • Code all diagnoses/procedures in compliance with DSM-IV and
ICD-9-CM
coding principles and manuals.
  • Assist HIM Technician, Correspondence Clerk, and Transcription Clerk as directed.
  • Meet productivity standards: coding at least three (3) medical records per hour for federally funded patients and six (6) medical records for non-federally funded patients.
  • Maintain quality standards: 95% accuracy in coding principal diagnoses and procedures, with 99% accuracy in abstracted information.
  • Notify the director when work is more than 48 hours behind schedule.
  • Assist the director with state requirements and reports.
  • Ensure data quality and optimize reimbursement under federal and state payment systems.
  • Serve as a coding resource for hospital staff and provide education on coding issues.
  • Stay familiar with all medical record and coding requirements.
  • Accept additional assignments as needed.
  • Maintain good working relationships within the department, other departments, and medical staff.
  • Participate in performance improvement activities through data collection and documentation review.
