In this dynamic role, you will provide essential support to patients and residents, ensuring their safety, comfort, and well-being. Whether working in assisted living communities, nursing homes, hospitals, or home care environments, you will play a vital part in delivering personalized care that meets each individual's unique needs. Your proactive approach and attention to detail will help foster a nurturing environment where residents thrive and feel valued. Responsibilities Assist residents with daily activities such as bathing, dressing, grooming, mobility, and feeding to promote independence and dignity Monitor vital signs including blood pressure, pulse, respiration, and temperature; document findings accurately using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems Administer medications and treatments following physician orders while adhering to HIPAA regulations and safety protocols Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities Perform medical procedures including catheter care, tube feeding, and other basic nursing tasks under supervision or according to scope of practice Maintain detailed medical documentation and records using systems like Epic or Cerner; ensure compliance with medical terminology standards and privacy laws Collaborate with healthcare teams to coordinate patient care plans and communicate changes in condition effectively Qualifications Prior experience in assisted living facilities, nursing homes, hospitals, or home & community care environments is highly preferred Knowledge of physiology, anatomy, medical terminology, and patient care procedures such as medication administration and vital signs monitoring Experience working with individuals with disabilities or special needs is advantageous Familiarity with EMR/EHR systems like Epic or Cerner; ability to accurately document medical records and procedures Certification or training in caregiving roles; CPR/First Aid certification is a plus Strong interpersonal skills with a compassionate approach to patient-centered care Ability to follow detailed instructions while maintaining flexibility in a fast-paced environment Join us in making a meaningful difference!
