  • Friday 8:00am-4:30 pm
Location:
Apopka, FL The role you'll contribute: Completes 12 hours of in-service training or education annually as required by regulations and policy. Follows the Plan of Care (POC) and notifies the Case Manager or Supervisor when patient needs are not met by the current POC. Identifies and communicates adverse or problematic findings to the Case Manager or Supervisor. Charts patient information according to professional standards. Functions as part of the patient's interdisciplinary care team. Other duties as assigned. Provides personal care to convalescent, chronically ill, or disabled individuals in their homes. Assists patients and caregivers in achieving optimal levels of self-care and independence. Performs duties assigned by Supervisor or Case Manager, including bathing, oral hygiene, nail care, foot care, skin care, pressure relief, dressing, feeding, and transferring patients. Takes accurate vital signs and reports any deviations from established patient parameters as per the Plan of Care (POC). Describes and demonstrates techniques to assist patients with activities of daily living. Utilizes agency Home Health Aide (HHA) guidelines for additional resources and reference.
Knowledge, Skills, and Abilities:
  • Able to communicate effectively in English, both verbally and in writing [Required]
  • Additional languages [Preferred]
Education:
  • High School Grad or Equiv [Required]
Field of Study:
  • or equivalent
  • Completion of a certified home health aide course
  • On-the-job instruction relative to specific patient needs and continuing in-service training, so that patient safety may be assured and the personal care and homemaking needs of the patient are adequately met
Work Experience:
  • Six months nursing assistant experience [Preferred]
