Job Summary We are seeking a compassionate and dedicated Home Health Aide (HHA) to provide exceptional in-home care and support to individuals with diverse healthcare needs. As an HHA, you will play a vital role in enhancing the quality of life for your clients by assisting with daily activities, monitoring health conditions, and ensuring their safety and comfort. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately Provide medication reminders and assist with medication administration following prescribed instructions while maintaining compliance with HIPAA regulations Support clients with specialized care needs such as Alzheimer's disease, dementia, memory care, hospice care, and working with individuals with developmental disabilities or disabilities Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure accurate patient records Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment Assist with medical procedures such as catheter care, wound care, or other treatments as directed by healthcare professionals Observe patient conditions closely and report any changes or concerns promptly to supervising healthcare providers Skills Strong knowledge of medical terminology and patient care best practices Experience working in assisted living facilities, nursing homes, or home health environments Proficiency in using EMR/EHR systems for documentation and communication Excellent caregiving skills with a focus on compassion and patience Ability to work independently while following detailed care plans and protocols CPR certification and knowledge of emergency response procedures Familiarity with medical equipment such as Hoyer lifts and blood pressure monitors Effective communication skills to collaborate with clients, families, and healthcare teams Ability to handle sensitive information securely in compliance with HIPAA regulations Experience providing long-term care or home & community-based services is highly desirable Join us as an HHA and become an essential part of a team committed to delivering personalized, respectful care that empowers individuals to live healthier, happier lives.
