Job Description

Overview Join our compassionate healthcare team as a Home Health Aide, where you will play a vital role in delivering personalized, high-quality care to individuals in their homes. This position offers an energizing opportunity to make a meaningful difference by assisting clients with daily activities, promoting independence, and ensuring their comfort and safety. As a Home Health Aide, you will work closely with patients, families, and healthcare professionals to provide comprehensive support that enhances quality of life. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, noting changes in health status and reporting findings accurately in medical records or electronic health record (EHR) systems Administer medications as prescribed, following proper procedures for medication administration and documentation Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities Operate medical equipment including Hoyer lifts, catheters, vital signs monitors, and other assistive devices to ensure patient safety and comfort Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation Maintain a clean and safe environment by assisting with light housekeeping tasks such as cleaning, laundry, and meal preparation in accordance with health standards Requirements Proven experience in home health care, senior care, long-term care facilities, nursing homes or assisted living communities Knowledge of medical terminology and familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation Certifications in CPR (Cardiopulmonary Resuscitation) and first aid are required; additional certifications such as hospice care or hospice certification are a plus Ability to perform patient observation accurately and respond swiftly to changes in condition or emergencies Experience working with individuals with disabilities or special needs is highly valued Strong communication skills to effectively coordinate with healthcare teams and family members Ability to handle physical tasks including lifting patients using Hoyer lifts and assisting with mobility needs safely Knowledge of HIPAA regulations to ensure patient confidentiality at all times Join us in delivering compassionate home health services that truly make a difference!
