Job Description

Job Summary Join our dedicated healthcare team as a Home Health Provider, where you will deliver compassionate, personalized care directly to patients in their homes. This vital role involves assessing patient needs, administering treatments, and supporting recovery and well-being in a familiar environment. You will play a key part in improving quality of life for individuals across diverse age groups and health conditions, ensuring they receive safe, effective, and respectful care. Your energetic approach and clinical expertise will help foster independence and comfort for those you serve. Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and functional evaluations to determine individual care needs. Administer medications via injections, IV infusions, or other methods following prescribed protocols while ensuring proper documentation. Provide specialized treatments such as wound care, suturing, catheterization, tube feeding, and sterile processing techniques. Assist with activities of daily living including mobility support using equipment like Hoyer lifts or wheelchairs, and help with personal hygiene routines. Manage complex medical equipment including ventilators, dialysis machines, or telehealth devices to monitor patient health remotely or on-site. Collaborate with interdisciplinary teams on discharge planning, case management, and utilization review to optimize patient outcomes. Maintain accurate medical records using EMR (Electronic Medical Records) systems such as Epic or eClinicalWorks while adhering to HIPAA regulations for patient privacy. Experience Previous experience in home care or outpatient settings with a strong understanding of medical documentation and patient assessment techniques. Hands-on experience with IV insertion, medication administration, phlebotomy, and sterile technique procedures. Familiarity with diverse healthcare environments including hospitals (ICU, emergency room), nursing homes, assisted living facilities, or hospice settings. Knowledge of clinical research protocols related to geriatrics, pediatrics (including neonatal ventilator management), or behavioral health populations. Exposure to specialties such as gastroenterology, cardiology (including cardiac catheterization), infectious disease care, or pain management is highly desirable. Proficiency in working with EMR/EHR systems like Cerner or Athenahealth; experience with CPT coding and ICD coding is a plus. Ability to work independently in patients' homes while demonstrating excellent communication skills and cultural sensitivity across diverse populations including children and seniors with disabilities.
