In this vital role, you will provide essential support in delivering high-quality patient care across various settings, including assisted living facilities, nursing homes, hospitals, and home care environments. Your expertise will help ensure the safety, comfort, and well-being of individuals with diverse needs, including those with developmental disabilities, dementia, or undergoing hospice care. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working within a dynamic healthcare team committed to excellence. Duties Assist with daily patient care activities such as vital signs monitoring, medication administration, and basic nursing procedures Support residents and patients with activities of daily living (ADLs), including mobility, hygiene, and feeding assistance Document medical information accurately using Electronic Medical Records (EMR) and Electronic Health Records (EHR) systems like Epic or Cerner Administer treatments such as tube feeding, catheter care, and other specialized procedures following physician orders and safety protocols Monitor patient physiology and respond promptly to changes in condition or emergencies Maintain compliance with HIPAA regulations by safeguarding patient confidentiality and medical records Collaborate with nursing staff and other healthcare professionals to coordinate comprehensive patient care plans Requirements Prior experience working in assisted living facilities, nursing homes, hospitals, or home care environments is highly preferred Knowledge of medical terminology, anatomy, physiology, and medication administration protocols Experience with medical documentation systems such as EMR/EHR platforms (e.g., Epic or Cerner) Familiarity with caring for individuals with dementia, Alzheimer's disease, or other memory-related conditions Ability to perform vital signs assessment and basic nursing tasks accurately and efficiently Strong understanding of HIPAA regulations and patient privacy standards Excellent communication skills to work effectively within multidisciplinary teams and with patients of diverse backgrounds Certification as a Med Tech or Nursing Assistant is required; additional certifications in hospice or dementia care are a plus Compassionate attitude combined with a proactive approach to patient safety and well-being Join us in providing exceptional care that truly makes a difference!
