Job Description

Join us as the Outpatient Services Coordinator at The Sanctuary at The Woodlands , where you will oversee the development and management of our outpatient behavioral health programs, ensuring excellence in care and service delivery.
Compensation & Benefits:

Schedule:
Full-time, Monday-Friday 8am-5pm Some evening or weekend hours may be required
Key Responsibilities:
Provide direct supervision to Peer Support Specialists under the guidance of the Director of Outpatient Services. Ensure compliance with legal and ethical standards for patient confidentiality and informed consent. Establish meaningful treatment plans and aftercare planning . Conduct individual and group therapy sessions, incorporating family members when appropriate. Participate in interdisciplinary team meetings to review treatment progress and address social and occupational barriers. Advocate for patients' rights, coordinate discharge planning, collaborate with external agencies and connect patients with community resources . Assist in program evaluation, including needs assessments, SWOT analyses , and quality improvement initiatives. Assist with initial intakes and
SLUMs Assessment Qualifications:
Master's degree in Social Work, Counseling, or a related field LMSW, ALC, LMFT, or LMHC licensure Strong organizational, interpersonal, and communication skills Experience in behavioral health, outpatient services, or intensive outpatient programs (IOP) preferred Why Join Us?
