Job Description

POSITION SUMMARY
Oversee all aspects of the funded program for individuals living with and affected by HIV. Responsible for monitoring patient outcomes.
PRIMARY ACCOUNTABILITIES
Achieve Results Coordinate all administrative aspects of the PHCP project. Oversee data management and interface with CRHC staff and community organizations. Operational Excellence Ensure all tasks provided and associated with patient care, patient administrative processes, and related duties complies with all regulatory and accreditation standards, as well as clinic policies and procedures. Relationships Develop favorable relationships with culturally diverse patients. Instruct patients and their family members training and/or information related to connection with prevention and awareness of HIV. Establish favorable working relationships with all staff members associated with center operations, including nurses, physicians and related staff. Stewardship and Professionalism Uphold and consistently represent the values and mission of the organization at all times. Represent the organization in a highly professional manner at all times. Ensure compliance and attention to all corporate policies and procedures.
PRIMARY TASKS & DUTIES
Collaborate with IT and Quality Nurse to document and audit all project and patient outcomes. Establish network within the culturally diverse community. Support patient self-management of disease and behavior modification interventions. Coordinate continuity of patient care with external healthcare organizations and facilities. Provide service in a manner that is appropriate for the patient's age; demonstrates knowledge and skills necessary to meet the patient's physical, psychosocial, educational, and safety needs. Conducts comprehensive, preventive screenings for patients and/or assists all support staff with patient interactions as needed. Follows through with post-visits duties. Other duties as assigned.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES
  • Read, write, and maintain patient records and related administrative documentation. Ability to chart data within electronic medical records software.
  • Ability to work with a diverse, sometimes difficult and demanding patient population
  • Adherence to risk management and quality programs.
POSITION REQUIREMENTS
Education Associate's Degree Experience Intermediate experience in Excel and data analytics Demonstrated success with clear thinking and ability to reorganize as needed. Demonstrated success in working independently, prioritization and problem solving. Demonstrated success in organization abilities. Demonstrated success in computer skills including ability to use computer for scheduling, word documents and reasonable keyboard skills. Demonstrated success in customer service/patient services or working with the general public, preferably in a medical care facility. Demonstrated success in managing difficult customer situations Significant knowledge of medical practices and procedures in a primary care environment. Physical/ Environmental Occasional lifting up to50 lbs. and pushing of 5-20 lbs. Sitting, standing and walking approximately 80% of the time A medium to high level of manual dexterity required Bending and reaching approximately 20% of the time Normal accessibility and mobility throughout the region required Normal overtime/extended work hours
