Job Description
Job Overview We are currently seeking a compassionate and reliable RN to assist with patient education, care coordination, and chronic disease management. This role is focused more on patient support and escalation guidance rather than hands-on clinical procedures. Responsibilities include educating patients on when follow-up care, provider visits, or ER evaluation may be necessary based on established protocols. An MD will be available on call, and training will be provided by the clinical team.
