JOB DESCRIPTION JOB SUMMARY
Under the direction of the Clinic Manager or Lead Physician, the Care Coordinator is responsible for facilitating patient care transitions, managing chronic disease registries, and ensuring patients receive timely access to necessary services. The Care Coordinator functions as a central point of contact for patients, bridging the gap between clinical encounters and home-based care to improve health outcomes and promote clinic efficiency.

Yes Duties/Responsibilities:
Facilitates patient transitions between care settings, including hospital discharges, specialty referrals, and home health services. Assists in the patient intake process by obtaining and recording medical history, medication lists, and identifying social determinants of health. Coordinates with patients to ensure adherence to care plans, including scheduling follow-up appointments and facilitating medication refills under provider direction. Acts as a primary point of contact for patients and families, providing education on disease management and navigating complex healthcare systems. Accurately documents patient encounters, care plan updates, and communication logs in the Electronic Medical Record (EMR) in a timely manner. Monitors and maintains patient registries to track compliance with preventive health screenings and chronic care quality metrics. Communicates effectively with internal care teams and external agencies to ensure seamless exchange of medical information. Performs other duties as assigned.
Required Skills/Abilities:
Clinical Proficiency:
Knowledge of medical terminology, basic pathophysiology, and understanding of standard clinical care plans.
Service Orientation:
Dedicated to discovering and meeting the needs of patients, ensuring a compassionate, patient-centered experience.
Organizational Skills:
Excellent time management and the ability to track multiple patient care pathways in a fast-paced environment.
Technical Skills:
Proficient computer skills and experience navigating EMR systems and patient scheduling software.
Professionalism:
Possess a strong work ethic, high level of professionalism, and strict adherence to HIPAA guidelines.
Language Ability:
Ability to explain complex health instructions to patients and families clearly; ability to draft professional communication for external care providers.
Reasoning Ability:
Ability to identify gaps in patient care, recognize when to escalate concerns to a physician, and interpret clinical guidelines furnished in written or oral form.
Teamwork/Service/Growth:
Strong commitment to collaborative care, a focus on meeting patient/customer needs, and a drive for continuous professional learning.
Education and Experience:
High school diploma or equivalent required; Associate degree in healthcare or related field preferred. Certification as a Medical Assistant (CMA/RMA), LPN, or equivalent clinical certification preferred. BLS/CPR certification for Healthcare Providers required. 1-2+ years of experience in a clinical, social work, or care coordination setting preferred.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Periodic standing and walking throughout the clinic to assist patients or consult with providers.
