Job Description

Chino Valley, AZ Schedule:
Monday-Friday | 8:00 AM-4:00 PM | 30 Minute or 1 Hour lunch Position Summary We are seeking a dedicated and organized Behavioral Health Care Manager to support our Collaborative Care Management (CoCM) program. This role is responsible for coordinating care, maintaining consistent patient follow-up, supporting treatment adherence, and ensuring smooth communication between patients and providers. The position is fully onsite in our office and involves frequent interaction with patients by phone and through telemedicine sessions. The ideal candidate is detail-oriented, patient-focused, comfortable working independently, and skilled in communicating with diverse populations. Primary Responsibilities Enroll eligible patients into the Collaborative Care Management program. Conduct monthly patient follow-up calls and ongoing outreach to track progress and address care gaps. Support providers through care coordination activities including scheduling, referral management, and care plan updates. Assist patients with telemedicine check-ins and ensure they are prepared for virtual provider visits. Document all patient interactions and case management activities in the EMR accurately and in a timely manner. Participate in interdisciplinary communication, including weekly case review meetings when applicable. Maintain compliance with CoCM processes, confidentiality requirements, and organizational standards. Perform additional administrative and care coordination duties as needed.
Qualifications:
High School Diploma GED Active Level 1 Fingerprint Clearance Card 2 forms of Valid Government ID Current First Aid & CPR certification TB test (within the last 12 months) Bachelor's degree in a behavioral health-related field OR minimum of 2 years of behavioral health experience (residential, inpatient, or outpatient) Familiarity with medical terminology Experience working with EMRs and Microsoft Office applications (EClinicalWorks experience is a plus) Skills & Competencies Strong verbal communication and excellent customer service Ability to multitask, organize workload, and manage time effectively Comfort speaking with patients over the phone and video platforms Ability to follow protocols, uphold policy, and maintain strict confidentiality Compassionate, patient-centered approach to care Ability to complete documentation timely and accurately Work Environment This role is fully onsite in an integrated primary care/urgent care medical office. Patient interaction occurs via phone calls, telemedicine support, and occasional in-person conversations.
