The ideal candidate will have strong clinical and administrative skills, excellent communication abilities, and experience working in a fast-paced pulmonary and sleep medicine environment. This role will assist with CCM/RPM patient management, phones, medication prior authorizations, bronchoscopy coordination, chart preparation, and occasional clinic coverage to ensure efficient daily operations and continuity of patient care. Essential Duties and ResponsibilitiesCCM/RPM Responsibilities Manage and coordinate Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) patients Conduct monthly patient outreach and follow-up calls Document patient interactions accurately within the EMR Monitor compliance and ensure timely completion of CCM/RPM requirements Coordinate care plans, referrals, and patient education Collaborate with providers and clinical staff to improve patient outcomes Clinical & Administrative Support Answer and manage incoming phone calls professionally and efficiently Assist with medication prior authorizations and prescription coordination Coordinate bronchoscopy scheduling, authorizations, and related documentation Perform chart preparation for upcoming patient appointments Assist with patient scheduling and follow-up appointments Provide occasional front and back-office clinic coverage as needed Maintain HIPAA compliance and patient confidentiality at all times Qualifications Minimum 3-5 years of Medical Assistant experience preferred Prior experience with CCM and/or RPM programs strongly preferred Experience with pulmonary, sleep medicine, or specialty practice preferred Knowledge of insurance verification and medication prior authorizations Strong EMR/Practice Management system experience Excellent multitasking, organizational, and communication skills Ability to work independently and collaboratively within a team environment Bilingual (English/Spanish) preferred Preferred Skills Strong patient engagement and customer service skills Ability to prioritize tasks in a high-volume environment Detail-oriented with strong follow-through Comfortable handling both administrative and clinical responsibilities
