Job Description

Home Health | Hospice | Caregiving | Pharmacy | Transportation Are you looking to join a dynamic team of care professionals? Billet Health is seeking a compassionate Patient Care Coordinator to join either our Intake team or Staffing team! Our dedicated, hardworking staff services all of Arizona, as well as parts of Nevada, California, New Mexico, and Texas. Our team works closely with patients to establish trusted relationships and ensure a seamless continuum of care. Position Overview The Patient Care Coordinator serves as a liaison between new patients and field staff in a fast-paced environment. Assisting with Home Health referrals as they come in across multiple regions. Key Responsibilities Accept and process Home Health referrals Verify insurance, eligibility, and patient demographics Maintain constant communication with providers, hospitals, patients, marketers, and family members Coordinate care between patients and field staff Ensure accurate and timely documentation Schedule Training Period (First 4 Months): Monday

PM Post-Training Schedule Options:
Friday
  • Monday, 8:00 AM
  • 6:00 PM OR Thursday
  • Sunday, 8:00 AM
  • 6:00 PM Qualifications Minimum 1-2 years of healthcare experience preferred (Home Health, Hospice, Medical Office, or Medical Assistant experience strongly preferred) Knowledge of medical terminology preferred Strong data entry and computer skills Proficiency with Microsoft Office, Google Workspace, and EMR systems Excellent organizational, multitasking, and time management skills Strong communication and customer service skills (including phone etiquette) Meticulous attention to detail Benefits 401(k)
