Adult SMI Services :
The Clinical Coordinator will train new case-management staff and serve as point person for procedural questions pertaining to the Adult SMI Program working out of our West Valley Behavioral Health Center.
Duties include:
Daily oversight of Case Management staff, monitoring and tracking referrals, ensuring coordination of care with SMI Clinics and outside stake holders, ensuring staff are documenting case management activities in the EMR/Next Gen, helping case management staff navigate the system. Provides clinical oversight and supervision of case-management services, including intake and assessment for adult SMI clients. Discharge planning and coordination with other agencies with a focus on SMI services, whole health integrated care and other agencies. Inquires about the client's health & wellness needs and assists in coordinating those primary care or other supportive services with appropriate JFCS staff or external parties to ensure the client's whole health is addressed.
Qualifications:
Bachelor's Degree in Social Work or related field is preferred; or a commensurate combination of experience and education 5+ years' experience as a direct service provider in a Behavioral Health setting. Supervision experience and experience working with the Adult system of care preferred and/or working with teams of case management staff Ability to obtain a Level One Fingerprint Clearance Card, without restrictions. Bilingual Spanish-English preferred; Pay Differential offered.
