Salary Range:

$115,000-$120,000/year (based on experience)

Director, Case Management Opportunity:

Schedule:

Days; Mon-Fri Director, Case Management oversees case management operations, including care coordination, discharge planning, and utilization review Leads and develops case management staff to support quality outcomes, regulatory compliance, and patient satisfaction Collaborates with physicians, nursing leadership, and interdisciplinary teams to optimize patient throughput and resource utilization

Director, Case Management Qualifications:

The Director, Case Management must have an LCSW or RN license Must have 3 or more years of hospital-based case management experience Have or be willing to obtain CCM or ACM Bachelor's Degree or higher required About the
