Shift:
Mon,Tue,Wed,Thu,Fri Join Chapa-De as a Referral Coordinator and play a key role in helping patients access timely specialty care and telehealth services. This position is ideal for someone who thrives in a fast-paced healthcare environment, enjoys coordinating care across teams, and is passionate about creating a positive patient experience.Hourly Rate:
$23.00 - $25.80 per hourBenefit Highlights:

Coordinate and process medical referrals, prior authorizations, and specialty care appointments while ensuring timely follow-up and accurate documentation within the EHR system. Serve as a liaison between patients, providers, specialists, and insurance carriers to support seamless communication and continuity of care. Support Chapa-De's telehealth services by scheduling appointments, coordinating eConsults, preparing equipment, and assisting patients and providers during virtual visits. Monitor referral workflows, insurance portals, and referral statuses to ensure quality, compliance, and an exceptional patient experience throughout the referral process.Experience and Skills:
Qualifications:
High School Diploma or equivalent. One year customer service experience in a healthcare environment using a medical EHR system with understanding of basic medical terminology. Prior experience answering phone calls and using computer-based applications to coordinate patient care. Familiarity with or prior use of Microsoft Office (ex: Outlook, Teams, Excel)About Us:
