In this vital role, you will provide comprehensive psychosocial assessments, support patient and family needs, and coordinate care plans to enhance overall well-being. Your expertise will help patients navigate complex emotional, social, and practical challenges associated with their health conditions. This paid position offers an exciting opportunity to make a meaningful impact in diverse clinical setting in an adult day care environment. The ideal candidate will bring a passion for person-centered care and a proactive approach to case management. Responsibilities Conduct thorough assessments to identify psychosocial needs, including mental health status, family dynamics, and social support systems. Develop individualized care plans. Collaborate with multidisciplinary teams to coordinate services for consumers with complex conditions like dementia, autism spectrum disorder, PTSD, or developmental disabilities. Manage medical documentation accurately within electronic medical records systems while ensuring compliance with HIPAA regulations. Facilitate group therapy sessions and conduct intake evaluations to determine appropriate treatment pathways. Qualifications Bachelor's degree in Psychology; Bachelor's degree in Social Work; Master's degree in Social Work or related field required. Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management . Experience working with diverse populations including seniors and individuals with disabilities or developmental disorders. Excellent communication skills with the ability to perform diagnostic evaluations and deliver psychotherapy effectively. Ability to handle crisis situations with professionalism and empathy while managing multiple priorities. Computer savvy
